Why are mortality rates in Africa – Africa Suharan so high?

I am writing an essay on this question "why why mortality rates in sub suharan so high and want to help. what I have so far is the problem with HIV and AIDS, lack of sanitation, drinking water or hygiene, but need some help. Could get some ideas from the topics below? Mortality is affected by age of the population (because if Africa was a very young country with 43% under 15 so surely the death rate would be lower?)
